Hospital KPIs: 20 Performance Indicators Every Hospital Manager Should Track
A balanced set of hospital performance indicators across operations, quality, finance, patients and people, and how to turn them into a dashboard.
In this article
- Track KPIs in five areas: operations, quality & safety, patient experience, finance and people.
- Choose a few KPIs per area for leadership; keep the detail for departments.
- Every KPI needs a definition, data source, owner and target.
- A balanced scorecard links KPIs to strategy so they drive decisions, not just reports.
Hospitals generate a lot of data, but leadership often still asks: how are we really doing? The answer is a focused set of hospital KPIs, the performance indicators that show whether the hospital is safe, efficient, financially healthy and trusted by patients. Below are 20 of the most useful, grouped into five areas, with formulas.
Operations KPIs
| # | KPI | Formula / definition |
|---|---|---|
| 1 | Bed occupancy rate | Inpatient days ÷ (available beds × days) × 100 |
| 2 | Average length of stay (ALOS) | Inpatient days ÷ discharges |
| 3 | ER door-to-doctor time | Median minutes from ER arrival to first physician assessment |
| 4 | Left without being seen (LWBS) | ER patients who leave before assessment ÷ total ER arrivals × 100 |
| 5 | OR utilization | Used OR minutes ÷ available (staffed) OR minutes × 100 |
| 6 | Surgery cancellation rate | Cancelled scheduled surgeries ÷ scheduled surgeries × 100 |

| 12 | Inpatient mortality (risk-adjusted) | Observed ÷ expected deaths, using a recognized risk-adjustment method |
Patient experience KPIs
| # | KPI | Formula / definition |

From KPIs to a hospital balanced scorecard
A list of KPIs becomes a management tool when it is linked to strategy. The balanced scorecard groups indicators into four perspectives (financial, patient/customer, internal processes, and learning & growth) and connects each one to a strategic objective. For example, the objective "faster access to care" might be measured by door-to-doctor time, LWBS rate and OR cancellations.
How to build a hospital KPI dashboard
- Pick 10–12 KPIs for the executive dashboard, balanced across the five areas.
- Write a definition sheet for each: formula, data source, frequency, owner and target.
- Show 12-month trends with green/amber/red thresholds.
- Allow drill-down by department, ward and specialty.
- Review monthly and agree actions, not just explanations.
